Katsuhira lebt in der fiktiven japanischen Stadt Sugomori City. Durch einen Vorfall hat er sein Schmerzempfinden verloren und driftet seither lethargisch und ziellos durchs Leben, ohne sich viel um sich selbst oder sein Wohlergehen zu kümmern. Eines Tages tritt seine Mitschülerin Sonosaki vor ihn und teilt ihm mit, dass er ausgewählt wurde ein Kiznaiver zu sein. Als solcher wird er Teil des Kizuna-Systems, durch das seine Schmerzen ab nun mit einer Gruppe seiner Mitschüler geteilt werden. Ziel des Systems ist es, durch dieses Teilen der Schmerzen eine friedlichere Gesellschaft zu schaffen, doch kümmert das die ohne ihre Zustimmung ausgewählten Testpersonen wenig. Diese sehen sich nun nicht nur mit dieser neuen, schmerzhaften Abhängigkeit von anderen konfrontiert, sondern bekommen auch seltsame Aufgabenstellungen vorgegeben, deren Sinn für sie nicht wirklich erkennbar ist.
